Admissions

We understand that circumstances leading to residential placement are both unique and often times, personal.  When the decision to place is made, the evaluation process is initiated in a sensitive and confidential manner. The admissions team will complete a thorough evaluation of all referral information, and in most cases conduct a pre-placement evaluation at a convenient, agreed upon location.

If appropriate, the admissions team will make recommendations for further evaluation through dinner visits, activities, or even sleepover visits to the home.  Following all planned visits, the Admissions Department will make a final determination, and the referring agency will be informed of a decision.  An inter-agency meeting will be scheduled to finalize placement planning.  All referrals are considered for services without regard to race, color, religious creed, disability, handicap, ancestry, national origin, sexual orientation, or age.

*All placement decisions are based on vacancy, appropriateness for program, and most importantly wellness of fit!

With each request for placement, the Admissions Department requires some pre-placement information which includes the following:

  • Medical history and insurance information.
  • A complete physical exam
  • TB screening by way of a current Mantoux test (within last 2 years). 
  • A psycho-social history to include family dynamics, presenting concerns, identified service needs, and treatment strategies. 
  • A current psychological evaluation.
  • A current psychiatric evaluation as needed.
  • Individualized Service Plan
  • Individualized Education Plan (School aged referrals only)
  • A completed application form. (see below for details)
  • All applicable consents.
